How to obtain a death certificate in Alaska

After a loved one passes away, obtaining a death certificate is one of the most important steps that needs to be taken. This document is crucial for arranging the funeral, carrying out organ donation, executing the will, and other such actions.

The process of obtaining a death certificate varies from state to state and even from county to county. To make the process easier, we have compiled the information you need to obtain a death certificate based on your location.

In Alaska, you can order a death certificate in person, through fax or through mail. First, download the application for an Alaska death certificate and fill out the form. If you decide to order the death certificate:

  • By Mail: Send the application to either of the following addresses:

  • In Person (Anchorage and Juneau Only): Fill out the form above and submit it in person to either of the two Alaska Vital Records Offices in Anchorage and Juneau, located at the same addresses as stated in the mailing option.

Note that there is a $25 fee associated with obtaining a death certificate in Alaska.
